• Complete a Confidential Financial Analysis CFA On-line Tuition Assistance Application  School Number:  68989
• A $20.00/$30.00 processing fee will be required.
• Fill out a signed Christian Scholarship Fund or  CSF Student/School Joint Scholarship Application for each enrolled child 
• and one  CSF Fund Income Worksheet per family.
• Please return the top two forms with a copy of your 2011 IRS 1040 tax return to the school office by April 18, 2012.
• If all this information does not give an accurate picture of your current or future financial situation, you are encouraged to give the school a letter explaining your circumstances.The school will take this information and make a recommendation to Christian Scholarship Fund. The actual monetary award will be given by Christian Scholarship Fund of Arizona. You and the school will be notified in writing the amount of the scholarship awarded.
